Am I the last one to know...
That you can add &fmt=18 to the end of a youtube url and get much higher quality video?

That only works if there is a higher quality version available.

There is a way you can tell, and it is also an alternate method for getting to the higher quality version. Bring up any video on YouTube itself (i.e., not a YouTube video embedded in another web page), and look immediately under the "Views" counter. If there is a higher quality version available, you will see a "watch in high quality" link.

Here is a random example.
